How To Prepare For Your Nutrition Assessment 

 

 

Before we meet you will need to fill out the following:

 

A detailed questionnaire, which will tell me about your medical and health

history, current lifestyle, and dietary practices.

 

A food journal, which will allow you to record your food consumption for 7 days.

Both are provided by me upon request.  Forms can be sent via email as an attachment, or they can be mailed to you. Send your request to me by filling out the contact form and state your preference.

 

Family consults are done the same way.

Fill out a client questionnaire for each member with the appropriate information. Be sure and list any diet restrictions, food likes and dislikes as well as specific goals that each person wants to accomplish. Also explain any lifestyle routines that limit the way your family eats such as working long hours, traveling, dining out, not enough time to cook, lots of schoolwork, activities, etc.

 

Food Intake and Activity Record

Print food log sheets for each family member and record food intake and activities for 7 days. Use the comment section for additional notes.  Also list each persons activity level for each day, Include any forms of exercise and any organized sports that a family member participates in. Be specific as to whether exercise is:

1. 30 minutes or less each day

2. 30 to 60 minutes of exercise each day

3. 60 minutes or more each day

 

After you have completed and returned the above paperwork, we set an appointment time to have your consultation. When we meet for your consultation, I will present the information provided to create a detailed nutrition and health assessment that we can then use to discuss the following:

                                                                                                                                              

Dietary plan for you and your family.

Tools to reach dietary goals.

Healthy nutrition and lifestyle changes.

Nutrition concerns and questions.

Meal planning and sample menu.

Supplement recommendations

Reading food labels.

Recipes and tips.

Opportunity to continue services.

The session usually takes anywhere from 45 minutes to 1 1/2 hours to complete.
